We are excited to have you as our patient! On your first visit, pay the one-time enrollment fee and first monthly payment and receive the included benefits of the plan you choose. Most plans include a comprehensive exam, x-rays, and a professional cleaning

We have a Perio Plan specifically designed for you. It includes your regular perio maintenance visits as well as additional discounts on specific perio services. During your initial visit, we will let you know if these services are needed.

Credit and Debit cards including MasterCard, Visa, American Express, and Discover are accepted. You can also use your HSA card.

Your preventive regular care includes a doctor exam, routine x-rays and a professional cleaning. These are included at no additional charge and are available based on the visit frequency you choose when you sign up for your plan.

Your plan includes discounts on restorative work that needs to be completed, (filling, crown, etc.). Please see your terms and conditions for detailed information about plan discount exceptions. This saves you money when work is needed. Some exclusions may apply.

Just schedule the work you need and we will automatically apply the discounts. There is no paperwork or hassle to receive the valuable discounts. Is there a waiting period for benefits and services? No! There are no limitations or waiting periods for current or future dental needs.

To encourage you to use your included preventative benefits, we will reward you with points that you can use toward any services you may need. Rewards accumulate after each Cleaning or Perio Maintenance visit and don’t expire as long as your membership remains active.

Yes! Individual rewards may be combined with other family members to maximize savings. Take advantage of having your entire family on the plan by using everyone’s rewards on a single procedure.

We plan on your membership continuing on a month-to-month basis for as long as you have teeth. The total monthly amount will be billed for the minimum time period and is not cancellable prior. If you need to cancel after the initial agreed upon time period give us a call and we will be happy to discontinue your plan. Detailed terms and conditions can be viewed and agreed upon during sign up.
